This form discharged Rifleman Frank Stupple of 9th Battalion The London Regiment (Queen Victoria's Rifles) as he was longer physically fit for war service. Stupple, who had enlisted in May 1915 and fought at Gommecourt on the Somme in 1916, was shot in the arm in 1917.
