This form states that if women are in difficulty while in London they should 'apply at Connaught Club Seymour Steet, London'. If they 'for any reason are unable to return from leave on the proper date will report the fact immediately and in writing to Headquarters, Q.M.A.A.C. 49 Grosvenor Street, London W1'.
